Why do people often think democracy and republic mean the same thing?

I understand that a monarchy can be classed as automatically undemocratic as the nation has an unelected head of state. However, the systems of constitutional monarchies such as the United Kingdom prove that they can be more democratic than several republics with dictatorial states such as Burma and North Korea functioning under a republican system.
